Dear Participant:

• Enclosed is your Clinical Chemistry specimen number ( ) tested negative for hepatitis and HIV .
• Keep the specimen lyophilized until use and store it at 2-8c.
• Before analysis open the vial very carfully to avoid any loss of lyophilized material then add exactly two ml (2 ml) of distilled water(DW),
• make sure that the serum lyophilized specimen is completely thawed and mixed gently at least for 20 minutes . Treat this specimen exactly as you do with any other clinical specimen(Handle as if potentially infectious).
• Complete the assay for analytes indicated on the accompanying Results Return Sheet.
• The tests are: Glucose, Creatinine, Urea,Uric Acid, Cholesterol, Triglyceride, Alkaline phosphatase,GOT,GPT, D.Bilirubin,CK,                                                              Amylase,Phosphorous,Mg,HDL-Cholesterol,LDL- Cholesterol,Pottasium,Iron,Sodium, Albumin & Total Bilirubin
• Use the methods and equipment reported in the methodology Registration sheet.
• On the Results Return Sheet record the date the sample was received and the date the sample was analyzed.
• Write your results clearly in the appropriate result space ensuring they are in the units indicated.
